It's from a book Liquitex used to publish called How To Mix And Use Color. There are several editions, that were sold both alone and with their Color Map and Mixing Guide (a poster-sized color chart that somewhat followed the Munsell system).

It's basically the Reilly palette... I always suspect that it's what Michael Whelan uses. I seem to recall an interview of him saying that he pre-mixed all his colors.

Do you find it applicable to oils and oil painting? Or is it matched up for use with Acrylics only?

Both, Liquitex had an oil line up to the early '90s, and they made the Color Map in both oil and acrylic versions.

In the sixties, Permanent Pigments developed a line of paints called the Modular Color System. It was essentially the same idea as the Reilly palette, adapting the Munsell system to oil and acrylic paints. There were premixed neutral grays, and an assortment of high-chroma colors at different values for each Munsell hue. This was the origin of the Color Map. Permanent Pigments was bought by Binny & Smith (Crayola) in the '70s, the oil line was rebranded with the now more well-known Liquitex name, and the Modular Colors were folded into the regular oil and acrylic lines. Up until the early '90s Liquitex still sold five values of gray (V3-V7), but today they only make V5. They are still one of the only paint manufacturers to include Munsell notation on their labeling, though.
